Assessments, Support Plans

I offer informative, strength-based, holistic, and person-centred assessments. These assessments incorporate consent for information sharing, risk assessment, and a consideration of health domains to triage for Continuing Healthcare (CHC)—please note, this is not the checklist. Separate Mental Capacity Assessments, Support Plans, and Reviews are also available. I can assist in finding care and signposting to relevant services, including GPs, Occupational Therapy, Physiotherapy, Continence Nurses, District Nurses, Mental Health Services, Fire, Police, third sector services, and Adult Social Care Services.

Sometimes, technology such as Telecare or Telehealth is all that is needed to stay safe at home. In other cases, moving to accommodation with staff onsite—such as Sheltered Accommodation or Extra Care/Assisted Living—may be the best option. We can discuss these options together to ensure you have time to make informed choices.

I can also advise on Personal Budgets, especially Direct Payments, and explain Care Act Eligibility and co-funding (Paying for Care).

Expert Witness

With over fifteen years of experience, I can provide reports or representation at Care Reviews, in court (as an expert witness), or for Independent Review Panels (Continuing Healthcare). This is a report-only service, ideal for those seeking someone deeply knowledgeable in social care—someone passionate and capable of conveying complex and ambiguous legalities to both individuals and other professionals, including the court. I am known for being thorough, clear, and knowledgeable in my field.

Mental Capacity Assessments

Mental Capacity Assessments are vital when making decisions about your present and future. They are also essential if you wish to give someone permission to make decisions should you ever lose capacity or if there are specific wishes regarding your care and support. I can assess and certify your ability to make particular decisions at the time of assessment. This is especially important if someone disputes your capacity in a specific situation—it is beneficial to have this documented in writing.

Lasting Power of Attorney

LPA is where you choose to legally empower a trusted person to make decisions on your behalf if you are ever unable to do so. This must be done while you are still mentally well. A doctor or mental health professional may be required to confirm your mental capacity. I can support you with the completion of LPAs and/or certify the Mental Capacity Assessment. The cost to register a Health or Wealth LPA is £82, or £164 for both, when applying directly through the Office of the Public Guardian, unless you qualify for a reduction.

Practice Educator

I hold a Full Practice Educator Award and support long-arm students already in placement. Colleagues describe me as patient and resourceful. My primary goal as a Practice Teacher is to facilitate student success, collaborating closely with universities and providing detailed reports. I excel at simplifying complex concepts, making it easier for students to apply theory in practice.

Continuing Healthcare

With extensive experience and a deep understanding of the CHC framework, I guide those with complex health needs through the complexities of health funding. Health can be described as the treatment and control of disease, illness, and disabilities, including aftercare. It is not a diagnosis alone that leads to a CHC award, but the type, quantity, and quality of care required. I am available to assist with screening, checklists, appeals at Independent Review Panels, and DST attendance (subject to availability). I also collaborate with law firms, local authorities, health authorities, and both private and voluntary sectors.

Hoarding

I have a particular interest in working with people who hoard and understand the various factors that contribute to this behaviour. My track record includes successfully preventing potential evictions. I recognise that simply clearing out a hoard is not a solution, as it can increase anxiety and exacerbate the behaviour. I approach this work with empathy, patience, and motivation, and am happy to take a hands-on approach. Organising possessions can be as gradual or as swift as you prefer. Local authorities may fund your support, but they often require evidence of progress. Independent support is advantageous, as I can work at your pace, particularly if you have a Personal Budget or are self-funding. With a Personal Budget in the form of a Direct Payment, you receive the funds and decide how your needs are met. I can provide a quote and plan to the funding authority so we can work together towards convenient and achievable outcomes.

I accept project work, self-referrals from self-funders, and referrals from:

  • Local authorities

  • GPs

  • Lawyers/law firms

  • NHS/ICB

  • Private healthcare providers/hospitals

  • Care homes

  • Domiciliary care/homecare agencies

  • Universities
